Performance Driven Case Management, LLC is a nurse case manager owned company that was founded on September 1, 2021 in the Tampa, Florida area. Our company was established by Tracey Canty, a caring, compassionate, and knowledgeable Registered Nurse Case Manager. Tracey has over 20 years of experience working with injured workers, adjusters, employers, medical providers, attorneys, and vendors. She has served as a dedicated case manager for several Fortune 500 organizations; managing construction, healthcare, hospitality, retail, technology, transportation and warehouse company accounts. She understands the unique challenges that the different accounts face in addressing the needs of their injured workers and navigating the insurance process. She takes pride in her ability to effectively communicate with all parties involved and resolve complex medical care issues. After her many years of working in the insurance industry, she decided to use her knowledge and skills to open her own case management company. Tracey desires to utilize her industry experience and competitive advantages to achieve optimal case management results.
